Resumo: Introdução: Objetivando a inclusão e buscando a diversidade que reflita a população, mudanças tem sido implementadas no ensino superior brasileiro. A Universidade Estadual de Campinas iniciou, em 2004, o Programa de Ações Afirmativas e Inclusão Social (Paais), um sistema de bônus na nota da segunda fase do vestibular, atribuindo 30 pontos aos alunos de ensino médio público e 10 pontos aos autodeclarados pretos, pardos ou indígenas. Há dúvidas sobre o desempenho de estudantes que tiveram benefício de ações afirmativas (AA). Objetivos: Avaliar a influência das características sociodemográficas, do recebimento do bônus e da bolsa estudantil sobre a validade preditiva do desempenho no vestibular em relação ao desempenho na graduação em medicina, e na seleção para os Programas de Residência Médica (RM). Sujeitos e métodos: Foi realizado um estudo de coorte retrospectiva com análise secundária dos dados dos alunos matriculados no 1º ano do curso de Graduação em Medicina entre os anos de 2005 a 2008, comparando o desempenho no vestibular com o coeficiente de rendimento no sexto (CR6) e no décimo segundo (CR12) períodos, e no processo seletivo para admissão à RM. Utilizou-se para análise a regressão logística e a multivariada, a One-Way Anova e o teste Post-hoc de Tukey, Correlação de Pearson, Chi-quadrado (X2) e teste T de Student, adotando 95 % de intervalo de confiança, o tamanho de efeito foi calculado com o eta-squared. Foram incluídos 417 estudantes de medicina, e 84 (20,1%) utilizaram bônus. Resultados: São apresentados quatro artigos, sendo o primeiro uma revisão bibliográfica sobre o tema das AA no acesso ao ensino superior. O segundo artigo mostra que o desempenho global no vestibular justifica 18% na variação do desempenho ao final do curso e 10% na seleção para a RM, enquanto português se correlaciona moderadamente com o CR12 e principalmente matemática, com o desempenho no exame para a RM. O terceiro artigo demonstra que os estudantes que receberam bolsas acadêmicas apresentaram renda familiar inferior e melhor rendimento acadêmico durante o curso médico. O quarto artigo mostra que os ingressantes com bônus eram mais velhos, com menor renda e escolaridade dos pais, menor CR6, porém desempenho similar no CR12 e RM. Os fatores associados ao CR6 e CR12, mas não à RM, na análise multivariada incluíram escolaridade materna, bolsa acadêmica, menor idade, e disciplinas do vestibular, exceto Física. Conclusão: Os estudantes com bônus concedido no vestibular finalizam o curso em condições equivalentes aos demais e com chances similares de admissão na RM. Sugere-se que o envolvimento com as atividades que permitem recebimento de bolsa durante a graduação, notadamente de iniciação científica, podem contribuir com a recuperação observada entre os estudantes de menor renda

Abstract: Introduction: Aiming at inclusion and seeking diversity that reflects the population, several changes have been implemented in Brazilian higher education, but there is a debate about the performance of students who benefited from affirmative action (AA) programs. In 2004, the State University of Campinas initiated the Affirmative Action and Social Inclusion Program (Paais), a bonus system applied in the second phase of the college entrance exam (CEE), offering 30 points for public high school students and 10 points for self-declared blacks, browns, or indigenous people. Objectives: To evaluate the influence of the sociodemographic characteristics, the receipt of the bonus and the student scholarship on the predictive validity of the performance in the college entrance exam in relation to the performance in medical graduation, and in the selection for the Medical Residency Programs (MR). Subjects and methods: A retrospective cohort study was carried out with a secondary analysis of the data of the students enrolled in the 1st year of the Medical Graduation course between the years 2005 and 2008, comparing the performance in the CEE with the coefficient of performance in the sixth (GPA6) and in the twelfth (GPA12) periods, and in the selective process for admission to MR. One-way ANOVA and Tukey's Post-hoc test, Pearson's correlation, Chi-square (X2) and Student's T-test were used for analysis, adopting 95% confidence interval, the effect size was calculated with eta-squared. 417 medical students were included, and 84 (20.1%) used bonuses. Results: The median age was 20 years (ranging from 17 to 33), with a predominance of females (55%). Four articles are presented in the results, the first one being a bibliographical review on the topic of affirmative actions (AA) in access to higher education. The second article shows that the grade of the first phase of the college entrance examination were associated with the performance during the graduation and that the overall performance in the CEE justifies 18% in the variation of the performance at the end of the course and 10% in the selection for MR, while Portuguese correlates moderately with GPA12 and mainly mathematics, physics and geography, with the performance in the MR exam. The third article shows that scholarship students had lower family incomes and improved academic performance during the medical course. The fourth article shows that bonus participants at medical school admission were older, had lower income and schooling of their parents, and presented lower GPA6, but similar performance for GPA12 and in the MR selection exam. Factor associated with GPA6 and GPA12, but not MR included maternal schooling, scholarship, lower age and most subjects for medical school admission. Conclusion: Students receiving bonus at medical school selection exam completed the course under equivalent conditions to others and similar chances of admission to MR. It is suggested that the involvement with activities that allow the scholarship to be received during graduation, particularly for scientific initiation, contribute to the observed recovery among lower-income students
