Abstract: The authors report a case of a patient where the finding of an apical ventricular aneurysm, identified in necroscopic and ventriculographic studies, has been thought to be due to akinesis of the apex cordis. The cause of the akinesis was an injury to the left conduction system by a mitral valve prosthesis. Attention is called to individual anatomic variations of the His bundle, to a frequent individualization of a third fascicle of the left bundle branch (the medio-septal fascicle) and to the anatomofunctional characteristics of the local Purkinje system that could change the mechanism of the apical depolarization.
