Before discharge (hospital) and after (home), the Patient Continuity of Care Questionnaire was applied, translated and adapted. Th ere was a significant correlation among subscales. Th e results reaffirmed the importance of planning and coordinating the hospital discharge to ensure continuity of care and provide comprehensive care in the field of elderly health. It is recommended to validate the instrument adopted in order to facilitate multicenter studies.|
