Title: IMIPENEN CILASTATIN IN THE TREATMENT OF PELVIC INFECTIONS

Abstract: Twenty-one female, hospitalized patients with diagnosis of moderate-to-severe pelvic infections were studied in an open, comparative clinical trial. Diagnosis were: inflammatory pelvic disease (N = 15), puerperal endometritis (N = 3), and infected abortion (N = 3). After clinical diagnosis (complete physical including rectal and gynecologic examination) and laboratory (creatinine, transaminases, urinalysis) and microbiologic specimens collections (urine, endocervical swab and secretions from suspected infection sources) were obtained for aerobic and anaerobic cultures, patients were randomly assigned to one of the two treatment groups. Group I was administered imipenem/cilastatin 500 mg every 6 hours for five to 14 days and group II was given ceftriaxone 1,0 g q.p.m. during the same period. Clinical reevaluation occurred daily during hospitalization and three to 5 days after discharge from hospital. Laboratory and bacteriologic tests were performed prior to and three to five days after treatment. Mean age of patients from group I was 27.0 years and from group II 28.4 years. The most frequent pathogens were Staphylococcus sp (36,4%), E. coli (27,3%), Neisseria gonorrhoease 13,6%), Enterococcus sp (9,1%), Streptococcus sp (9,1%) and Klebsiella sp (4,6%). All cases showed a good clinical outcome and complete remission of symptoms and signs. The cure index of group was 100% and of group II 90%. All patients presented excellent tolerability in group I and one patient from group II developed "rash", that completely disappeared after treatment discontinuation. The present study confirms the high effectiveness and good tolerability of imipenem/cilastatin in the treatment of pelvic infections.
