Title: Fatores prognosticos associados ao tratamento inicial em pacientes com carcinoma do ovario estadios II, III e IV

Abstract: Resumo: O objetivo deste estudo foi avaliar certas características da paciente, da doença e da terapêutica como fatores associados à resposta ao tratamento inicial e à sobrevida em 65 pacientes portadoras de carcinoma de ovário estádios 11, 111 e IV, atendidas no Centro de Atenção Integral à Saúde da Mulher de janeiro de 1986 a fevereiro de 1994. Para avaliar a associação entre a idade, "performance status", variáveis histopatológicas, ascite, extensão da doença, tipo de cirurgia realizada, volume tumoral residual pós-cirúrgico e quimioterapia com a resposta ao tratamento inicial e a sobrevida foram realizadas análises univariadas e múltiplas. Após análise múltipla, a idade, ascite, . número de ciclos de quimioterapia e intervalo entre os ciclos foram fatores significativamente associados à resposta inicial. Em pacientes clinicamente livres de neoplasia, o volume tumoral residual esteve intimamente correlacionado com o achado de doença na cirurgia de revisão. Em relação aos fatores associados à sobrevida, além da idade, ascite, volume tumoral residual e intervalo entre os ciclos de quimioterapia, a resposta ao tratamento inicial definiu o prognóstico

Abstract: The main purpose of the present study was to analyze some characteristics of the patient, the disease and the treatment as associated factors with primary terapeutic response and survival in 65 patients with ovarian carcinoma stage li, 111 and IV, who were attended in the Center of Integral Assistance to Women's Health, from January 1986 to February 1994. To evaluate the association between age, performance status, histopathologic variables, ascite, disease extension, technical surgery, volume of residual tumor after debulking surgery and chemotherapy with the treatment resPQnse and survival both univariate and multiple analyses were used. After mulfiple analyses, age, ascite, number of chemotherapic courses and interval between these courses were significant factors associated with primary response. Regarding second look surgery in patients clinically free of neoplasia, the volume of residual tumor was directely correlated with the presence of disease. And, concerning factors related to survival, besides age, ascite, volume of residual tumor after debulking surgery and interval between chemotherapic courses, the response of primary treatment itself defined the prognosis
